Program Manager, Transitional Recovery Housing: We are looking for a committed, energetic leader who is a passionate advocate for housing as a human rights issue. The Program Manager will be responsible for overseeing evening and weekend operations of our Transitional Recovery Housing and supervision of Resident Advocates. This program works with single adults and families who are currently working towards sobriety and life-long goals while residing in Avivo’s Recovery Housing apartments.
Chemical Health Assessor: This person will conduct chemical health assessments with individuals accessing care at Avivo and facilitate admission to 245G programs. They’ll ensure that all individuals receive access to appropriate services and assistance in gaining access to services. This person will work in a dual capacity between the 1900 Chicago Avenue campus, Avivo Village (as well as other internal programs), and other community partners to increase client opportunities to direct access. Schedule: Monday –Friday (7 a.m. -3:30 p.m.), with the ability to work a flexible schedule.
Case Manager, Family Housing Stabilization Services (second shift): This person will work with mothers who are in Avivo’s Family Intensive Outpatient Programming for Dual Disorder Treatment, who are residing in Avivo’s family transitional recovery housing. They’ll hold their own caseload, supporting program participants to maintain their housing, assist them with their housing transition plan, and meet their individual service plan goals.
