Chemical Health Assessor: This person will conduct chemical health assessments with individuals accessing care at Avivo and facilitate admission to 245G programs. This role will ensure that all individuals receive access to appropriate services and assistance in gaining access services. This role will work in a dual capacity between the main campus on Chicago Avenue, Avivo Village (as well as other internal programs), and other community partners to increase client opportunities to direct access.
Clinical Program Manager, Avivo Village: This person is responsible for the clinical supervision of the Rule 29 clinic at Avivo Village. This leadership position is responsible for the development and coordination of Rule 29 services for adults with serious and persistent mental illness who are experiencing homelessness and have a possible chemical health addiction. Avivo Village is an alternative to encampments that creates a safe, secure, and service-rich environment for unsheltered individuals in hopes of securing long-term housing. Avivo has a Housing First and Harm Reduction approach and maximize access to other services within Avivo.
Chemical Health Professional, LADC, Family Treatment: This person will provide clinical treatment counseling and case management to women as part of the Family Treatment Team in Avivo’s licensed 245G treatment program (outpatient). This unique program allows pregnant and parenting mothers (with children up to age 17) to enter treatment as a family. This role includes facilitation of clinical services, both individual and group sessions. This individual will work as part of the multi-disciplinary Family Treatment Team.
